The notification of an insurance claim (following a written claim for damages against an insured person or company, or due to circumstances which could lead to such claim) has to be done according to the conditions defined in the policy. This form can help the insured to collect the necessary information for reporting a claim to the insurer. There is however no obligation on the part of the insured to use this form for such notification. Certain insurers may provide their own forms.
